Home Care

Stay in your own home with professional support, safely and comfortably.

Home care takes place in your loved one's residence and is ideal for seniors who need support but want to remain at home. Private-duty companions and aides can assist with personal care, meal preparation, errands, doctor visits, light housekeeping, shopping, and social companionship.

Note: Home care aides do not provide skilled nursing care. Average age served: All ages. This is often the first step families consider, and the right answer for many situations.

Independent & Retirement Communities

A vibrant social life in a supportive community, for active seniors who want more connection.

Designed for active seniors 55+ who are largely independent, these communities offer private apartment-style units, often with full kitchens, in a vibrant social setting. With 50 to 200+ residents, they typically include meals, housekeeping, transportation, entertainment, and activities.

Best for: Seniors who are healthy and independent but want social connection, convenience, and a maintenance-free lifestyle.

Assisted Living

Get the help you need with daily life, in a warm community, without giving up independence.

For seniors who need more support with daily life, assisted living communities provide a safe, social environment with trained staff. Private and shared units are similar to senior apartments. Includes meals, housekeeping, transportation, activities, supervision, and assistance with daily activities and medication management.

Typical community size: 50 to 200+ residents. Many families are surprised by how much they feel like home.

Memory Care

Specialized care for Alzheimer's, dementia, and Parkinson's, with trained staff and purpose-built environments.

Secure, specialized communities for seniors with Alzheimer's, Parkinson's, dementia, or other forms of memory impairment. Smaller in scale (20 to 50+ residents), these communities feature specially trained staff, purpose-built environments, and activities designed to stimulate and support residents with memory challenges.

24-hour awake staff provided. We help families understand when memory care is appropriate and how to evaluate quality of care.

Continuing Care Retirement (CCRCs)

One community for life — from independent living through skilled nursing, so your loved one never has to move again.

CCRCs offer a full continuum of care on a single campus, from independent living to assisted living to skilled nursing, so your loved one can age in place without having to move as their needs change. Ideal for seniors and families who want long-term stability and peace of mind.

CCRCs often have a BUY-IN fee.

Residential Care Homes

Highly personal, attentive care in a true home setting, often at a lower cost than a large facility.

A warm, home-like setting for seniors who need more hands-on care in a smaller environment. With just 1 to 6 residents, these homes offer highly personalized attention at a cost-effective price point. Includes meals, housekeeping, activities, supervision, personal care, and medication assistance.

Often overlooked, residential care homes are for families who want a more intimate, home-like environment.

Respite Care

Get the break you need as a caregiver, while your loved one is safe, comfortable, and well cared for.

Respite care provides short-term relief for family caregivers. We help families find temporary care solutions that give caregivers a needed break while ensuring their loved one is safe, comfortable, and well cared for.

Family caregiver burnout is a real and serious issue. Respite care is not a luxury, it's a necessity for sustainable long-term caregiving.

Hospice Care

Comfort, dignity, and support for both your loved one and your family during the most difficult time.

Hospice care focuses on comfort, dignity, and quality of life for seniors with a terminal illness. We help families understand their options and connect with compassionate hospice providers.

Hospice is often chosen later than families wish they had. If a doctor has mentioned it, it's worth understanding the full picture now rather than in a crisis moment.

How does A Plan For Senior Care get paid?

A Plan For Senior Care is compensated by the licensed facilities and agencies we represent. Those facilities are prohibited from charging referred individuals more because of our involvement, and we do not solicit, receive, or accept gifts or referral incentives from any facility or agency we work with.

What's the difference between assisted living and memory care?

Assisted living communities support seniors who need help with daily activities (meals, medication, personal care) while still maintaining significant independence. Memory care communities are specifically designed for seniors with Alzheimer's, dementia, or other cognitive conditions. They have secured environments, specially trained staff, and programming built around cognitive support. We help you understand which is appropriate and when, based on your loved one's current and likely future needs.

How do we know when it's time to consider senior care?

Common signs include difficulty managing medications, increased falls or near-falls, confusion or memory lapses, difficulty preparing meals, neglect of personal hygiene, or social withdrawal. Many families wait longer than they should. The earlier you explore options, the more choices you have. We're always happy to answer a "just checking" call, even if you're not sure care is needed yet.

We're not local to Southern California. Can you still help?

Yes, we assist families throughout California. Many of the adult children we work with live outside of Southern California and are managing this process remotely. We can handle research, phone consultations, and coordination on your behalf. For local tour accompaniment, we assist families in the greater Los Angeles and Orange County areas, but we work with out-of-area families by phone and email regularly.
