Initial Consultation & Needs Assessment
In our first conversation, we learn about your loved one's unique situation and answer your questions. We take time to ease your stress before we get into details. During our assessment, we listen carefully and gather key information:
- Physical care needs: mobility, medical conditions, medication management, cognitive status
- Financial budget: what is available, what may be available through insurance or benefits
- Location preferences: proximity to family, preferred neighborhoods or communities
- Social needs: activities, community size, cultural preferences, lifestyle priorities
Evaluation & Care Planning
After the assessment, we evaluate all of the information together. We identify the type of care that best fits your loved one's current needs, and anticipate future needs as well, so the plan holds up over time. This is where our experience matters most: matching care needs to the right level of support, in the right environment, at the right price point.
Your Plan
We create a personalized care plan based on your loved one's specific criteria. We include additional resources for anything beyond placement (like legal, financial, or medical) and we review the full plan with you together.

Navigating Care Options

How does A Plan For Senior Care get paid?
A Plan For Senior Care is compensated by the licensed facilities and agencies we represent. Those facilities are prohibited from charging referred individuals more because of our involvement, and we do not solicit, receive, or accept gifts or referral incentives from any facility or agency we work with.
What's the difference between assisted living and memory care?
Assisted living communities support seniors who need help with daily activities (meals, medication, personal care) while still maintaining significant independence. Memory care communities are specifically designed for seniors with Alzheimer's, dementia, or other cognitive conditions. They have secured environments, specially trained staff, and programming built around cognitive support. We help you understand which is appropriate and when, based on your loved one's current and likely future needs.
How do we know when it's time to consider senior care?
Common signs include difficulty managing medications, increased falls or near-falls, confusion or memory lapses, difficulty preparing meals, neglect of personal hygiene, or social withdrawal. Many families wait longer than they should. The earlier you explore options, the more choices you have. We're always happy to answer a "just checking" call, even if you're not sure care is needed yet.
We're not local to Southern California. Can you still help?
Yes, we assist families throughout California. Many of the adult children we work with live outside of Southern California and are managing this process remotely. We can handle research, phone consultations, and coordination on your behalf. For local tour accompaniment, we assist families in the greater Los Angeles and Orange County areas, but we work with out-of-area families by phone and email regularly.
